Case management in Kharkiv and Sumy regions: comprehensive support for recovery after difficult life circumstances

The war changed the lives of thousands of people in Kharkiv and Sumy regions. Loss of jobs, housing, documents, psychological trauma are just some of the challenges our fellow citizens face. Case management is our response to the need for an individual comprehensive approach to each situation.

As part of the project, we provide three types of case management:
- Psychological support and support
- Social support and support
- Support and support in cases of gender-based violence

Mobile team specialists process applications or independently identify individuals who need case management services, conduct an initial assessment of needs, form an individual support plan, and provide referrals to appropriate services.

Support for gender-based violence cases is carried out in accordance with international standards, taking into account informed consent, adherence to the principles of safety, confidentiality, dignity, and non-discrimination. An individual action plan is developed for each case, regular counseling, support, and, if necessary, referral to medical, legal, and social services are provided.

Case management is about transforming vulnerability into strength through a comprehensive, person-centered approach to each story.
